Coaching Consent

Acknowledgement, Confidentiality and Participant Consent For Professional Coaching Services by Tomorrow Mind PTY LTD.

As part of providing professional coaching services to you, Tomorrow Mind PTY LTD needs to collect and record personal information from you that is relevant to your situation, such as your name, contact information, professional details, and other relevant information as part of providing services to you. This collection of personal information will be necessary to inform content and discussions throughout professional coaching sessions.

Your personal information is kept securely and, in the interests of your privacy, used only by staff from Tomorrow Mind PTY LTD and the authorised personnel, such as consultants (as necessary). Your personal information is retained to document what happens during sessions and enables Tomorrow Mind PTY LTD to provide relevant and informed Coaching to you. Consequences of not providing personal information

If you do not wish for your personal information to be collected in a way anticipated by this letter or the Privacy Policy, Tomorrow Mind PTY LTD may not be in a position to provide its services to you. You may request to be anonymous, or use a pseudonym unless it is impracticable for Tomorrow Mind PTY LTD to work with you, or if Tomorrow Mind PTY LTD is required or authorised by law to deal with identified individuals. In most cases, it will not be possible for you to be anonymous or to use a pseudonym.

Access to client information

At any stage, you are entitled to access your personal information kept on file, subject to exceptions in the relevant legislation. A representative may discuss with you different possible forms of access.

Disclosure of personal information

All personal information gathered by Tomorrow Mind PTY LTD during the provision of workshops and coaching services will remain confidential except when:

1. It is subpoenaed by a court; or

2. Failure to disclose the information would in the reasonable belief of Tomorrow Mind PTY LTD place you or another person at serious risk to life, health or safety; or

3. Your prior approval has been obtained to:

  a. Provide a written report to another professional or agency. e.g., a GP or a lawyer; or

b. Discuss the material with another person, eg, a parent, employer, or health provider; or

c. Disclose the information in another way; or

4.  You would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your GP) and disclosure of your personal information to that  third party is for a purpose which is directly related to the primary purpose for which your  personal information was collected; or

5. Disclosure is otherwise required or authorised by law.

On occasions, Tomorrow Mind PTY LTD may request your written consent to pass on specific information to managers (if relevant) should it be mutually agreed that this is beneficial for the participant and organisation. Your specific personal information is not disclosed to others, unless you consent or such disclosure is otherwise required by law. Your personal information will not be used, sold, rented, or disclosed for any other purpose.


Online Videos and Emailing

The delivery of Tomorrow Mind’s training and coaching services is provided in person or online via video calls. Tomorrow Mind PTY LTD requests your permission to provide services via third-party platforms. By signing the consent below, you:

  • Permit to the use and/or transfer of information for services via video as outlined above;

  • Will not hold Tomorrow Mind PTY LTD liable for any error(s) incurred by a third party platforms (including errors relating but not limited to transmissions, privacy, connectivity etc.);

  • Will not record, reproduce, or disperse any information from coaching sessions without prior written consent from Tomorrow Mind PTY LTD;

  • Will not reproduce or disperse material presented through coaching unless Tomorrow Mind PTY LTD stipulates it as a resource to share.

  • Will ensure that reliable connectivity of internet connectivity is available to conduct the session. Tomorrow Mind PTY LTD takes no responsibility for service or connectivity errors during the allocated sessions.

Connectivity and Confidentiality During Coaching Sessions

  • Ensure availability of latest technologies including connectivity, software, computer, microphone, speakers etc. 

  • Agree to session recordings, including through the use of AI, used confidentially and exclusively to enhance coaching sessions and follow up.

  • Will notify representatives from Tomorrow Mind of all persons within your room and/or listening prior to the session or at any time should additional people enter the room.

In-Person Service

On occasions, you may request in-person sessions to be provided to you and your organisation, which will come at a further cost. Confidentiality and disclosure of personal information apply as outlined above (Disclosure of personal information).   

Cancellation Policy

If, for some reason you need to cancel or postpone attendance to a scheduled coaching session, please give staff from Tomorrow Mind at least 24 hours notice. One (1) make-up coaching session is provided with each package purchased. Dates and times for coaching sessions will be booked in advance through Calendly.

Behaviour Expectations

In order for Tomorrow Mind PTY LTD to provide the highest quality of Professional Coaching, it is expected that participants will treat the Coach with respect. We have a zero tolerance for aggressive behaviour. We thank you in advance for your support in creating a safe place.

I have read and understood this Consent Form. By recording my details below, I agree to the above conditions for the services provided by Tomorrow Mind PTY LTD in relation to myself. All questions have been answered to my satisfaction. I agree to these for the services provided by Tomorrow Mind PTY LTD.
